They count the whole day total. From hole 1 all the way to hole 72 on the fourth day. If a course is par 72 then the total would be 288 minus any birdies eagles a player gets so if a player shoots 10 under par for the 4 day total it would give them a score of 278 for the tournament. Hope this helps!
